Specialized Short-term Rehabilitation

If you’ve suffered a heart attack, had bypass surgery, valve replacement surgery, or struggle with endurance, you need specialized care to get you back to your best. The myHeart™ Cardiopulmonary Rehabilitation Program provides short-term, sub-acute rehabilitation to rebuild the strength and confidence you need to bridge the gap between hospitalization and home.

Our highly skilled medical staff includes physicians, registered nurses, physical/occupational therapists, and registered dietitians who specialize in helping patients recover from the following surgeries and conditions:

  • Recent heart attack
  • Heart surgery
  • Heart valve replacement
  • Angioplasty, stent, and pacemaker placement
  • Thoracic surgery
  • Congestive heart failure
  • Angina/Arrhythmia

Orthopedic Rehabilitation Program

Specialized Short-term Rehabilitation

Whether you’ve had elective joint replacement surgery or need to recover from a traumatic injury, our orthopedic rehabilitation program can help get you back on your feet.

A skilled, multidisciplinary staff consisting of physicians, registered nurses, physical/occupational therapists, and registered dieticians provides short-term, sub-acute rehabilitation care to help you bounce back from the following surgeries and conditions:

  • Post-surgical hip, knee, joint replacement/repair
  • Orthopedic injury and multiple or traumatic fractures

Stroke Recovery Program

Having a stroke can be devastating, not only for the patient but also for their family. We are committed to helping patients and their families progress toward recovery by providing state-of-the-art, comprehensive rehabilitative care following hospitalization for stroke and other neurological disorders.

Our interdisciplinary care team includes physicians, registered nurses, physical, occupational and speech therapists, and registered dietitians who provide care to assist in recovery from stroke, hemorrhaging stroke and transient ischemic attacks (TIAs).

Our therapists are skilled in stroke care. We have specialists in functional vision, speech and the management of swallowing deficits. We also have therapists specialized in neuro-developmental training. We are trained in the use of state-of-the-art technology, including flexible endoscopic evaluation for swallowing (FEES), surface electromyography (SEMG) for swallowing therapy, and electrical stimulation.  Physical, occupational and speech therapy is available seven days per week.

Stroke recovery care focuses on maximizing the patient’s health and independence in such areas as:

    • Activities of living, such as eating, dressing, grooming, bathing, and homemaking
    • Communication with spoken and written words
    • Improving memory and judgment
    • Balance, coordination and overall mobility
    • Swallowing
    • Individualized exercise plan based on patient’s abilities
    • Self-image
    • Patient and family education

Our stroke recovery care provides treatment to help patients return to meaningful lives through relearning the skills necessary for everyday living. Our goal is to help patients achieve positive outcomes and to help them reach his or her highest level of functioning and independence in order to return to the community safely and with any necessary support services.

Medically Complex Program

Many people require skilled nursing and rehabilitative services following hospitalization for surgery, infectious disease conditions or other complex health conditions.  Many of these people require IV medications, complex wound care, close nursing and medical monitoring, and patient and family training to learn how to manage new medical devices, medications and diseases.

The Transitional Care program at the Jewish Home provides state-of-the-art, comprehensive rehabilitative, nursing and medical care following hospitalization.  Our staff is skilled in caring for patients with:

  • Intravenous medications
  • PICC lines
  • Dialysis
  • Diabetes
  • Wound care
  • Tube feedings
  • Colostomies
  • Respiratory needs
  • Multiple Drug Resistant Organism infections, incl. ORSA and MRSA

A Providers’ Overview of Short-term Rehabilitation at the Jewish Home

There are a lot of options out there when it comes to recommending a short-term rehabilitation program for your patients. And in their eyes, the facility you endorse is a direct reflection upon you.

When you choose Transitional Care at the Jewish Home, you can rest assured that your patients are receiving the high-quality medical care they need to recover in a comfortable, nurturing setting. We offer multiple specialized areas of care, including Cardiopulmonary, Orthopedic, Stroke Recovery, and Medically Complex. The Jewish Home is one of the few area facilities accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF).

A Collaborative Approach to Wellness

We understand the road back from a serious medical issue to independence is as much a mental journey as it is a physical one. Which is why our short-term rehabilitation programs are built around providing highly personalized, effective medical treatment in a hospitality setting.

Our on-site personnel include board-certified physicians in geriatrics and internal medicine, nurse practitioners, physician assistants, as well as physiatrists. Along with a 24-hour nursing staff and certified physical/occupational therapists, our entire medical team collaborates to develop plans specifically focused on each individual patient’s unique needs and goals.

We work around patients’ schedules and preferences to deliver a wide range of services and programs designed to reinvigorate and rejuvenate:
