Short-term Rehabilitation Services at the Jewish Home Farash Tower

Whether it’s illness, surgery, stroke or heart failure, the time after a serious medical issue can be concerning. The road back to independence is as much a mental and emotional journey as it is a physical one, which is why our short-term rehabilitation programs are built around providing highly personalized, effective medical treatment in a hospitality setting. So you can recover quickly in comfort and get back to being you.

If you need short-term rehabilitation after a hospital stay, we offer a range of specialized care programs to patients of all faiths and ethnicities. Each program is supported by a focused, highly qualified on-site medical staff. These include the following:

myPath™ Stroke Recovery Program

Having a stroke can be devastating, not only for the patient but also for their family. We are committed to helping patients and their families progress toward recovery by providing state-of-the-art, comprehensive rehabilitative care following hospitalization for stroke and other neurological disorders.

Our interdisciplinary care team includes physicians; registered nurses; physical, occupational and speech therapists; and registered dietitians who provide care to assist in recovery from stroke, hemorrhaging stroke and transient ischemic attacks (TIAs).

Our therapists are skilled in stroke care. We have specialists in functional vision, speech and swallowing deficits and who have received neuro-developmental training. We are trained in the use of state-of-the-art equipment, including flexible endoscopic evaluation for swallowing (FEES), surface electromyography (SEMG) for swallowing therapy and electrical stimulation. Physical, occupational and speech therapy are available seven days per week.

Stroke recovery care focuses on maximizing the patient’s health and independence in such areas as mentioned below:

  • Activities of living, such as eating, dressing, grooming, bathing and homemaking
  • Communication with spoken and written words
  • Improving memory and judgment
  • Balance, coordination and overall mobility
  • Swallowing
  • Individualized exercise plan based on patient’s abilities
  • Self-image
  • Patient and family education

Our stroke recovery care provides treatment to help patients return to meaningful lives through relearning the skills necessary for everyday living. Our goal is to help patients achieve positive outcomes and to help them reach their highest level of functioning and independence in order to return to the community safely and with any necessary support services.

myHeart™ Cardiopulmonary Rehabilitation Program

If you’ve suffered a heart attack, had bypass surgery, valve replacement surgery or struggle with high blood pressure, you need specialized care to get you back to your best. The myHeart™ Cardiopulmonary Rehabilitation Program provides short-term, sub-acute rehabilitation to rebuild the strength and confidence you need to bridge the gap between hospitalization and home.

Our highly skilled medical staff includes physicians, registered nurses, physical/occupational therapists and registered dietitians who specialize in helping patients recover from the following surgeries and conditions:

  • Recent heart attack
  • Heart surgery
  • Heart valve replacement
  • Angioplasty, stent and pacemaker placement
  • Thoracic surgery
  • Congestive heart failure
  • Angina/Arrhythmia
  • COPD–bronchitis, emphysema, bronchiectasis and/or asthma
  • Pneumonia
  • Lung cancer
  • Disorders requiring CPAP and biPAP, including obstructive sleep apnea
  • Respiratory infection requiring IV antibiotic therapy

Orthopedic Rehabilitation Program

Whether you’ve had elective joint replacement surgery or need to recover from a traumatic injury, our orthopedic rehabilitation program can help get you back on your feet.

A skilled, multidisciplinary staff consisting of physicians, registered nurses, physical/occupational therapists and registered dieticians provides short-term, sub-acute rehabilitation care to help you bounce back from the following surgeries and conditions:

  • Post-surgical hip, knee, joint replacement/repair
  • Spine surgery or vertebral fusion
  • Osteoporosis
  • Degenerative joint disease

Medically Complex Program

Many people require skilled nursing and rehabilitative services following hospitalization for surgery, infectious disease conditions or other complex health conditions. Many of these people require IV medications, complex wound care, close nursing and medical monitoring, and patient and family training to learn how to manage new medical devices, medications and diseases.

In the Weinberg Manson Transitional Care program we provide state-of-the-art, comprehensive rehabilitative, nursing and medical care following hospitalization for these medically complex conditions. Our staff is skilled in caring for patients with the following:

  • Intravenous medications
  • PICC lines
  • Dialysis
  • Diabetes
  • Wound care
  • Tube feedings
  • Colostomies
  • Respiratory needs
  • Multiple Drug Resistant Organism infections, including ORSA and MRSA.
