Regaining Independence

The Atkin Center for Outpatient Rehabilitation provides multi-disciplinary rehabilitation services for adults who have more complex medical issues, assisting them in remaining in their own homes. With a spectrum of therapy services, as well as specific treatments for a range of health issues, participants visit us for the therapy they need to return to their highest level of independence. Our experienced therapists work in a team approach in order to provide comprehensive support. We offer physical, occupational, speech & language therapies in one location to support recovery and build patient strength and confidence to get back to fuller functioning and living.

Chad Mapes, MSPT
Director, Atkin Center for Outpatient Rehabilitation

Chad directs all aspects of the outpatient rehabilitation department, which includes such clinical services as physical therapy, speech therapy, occupational therapy, certified hand therapy and massage.

Chad earned his Bachelor of Science and Master of Science of Physical Therapy from D’Youville College in Buffalo, NY. His post graduate studies have concentrated in the area of manual orthopedic physical therapy with special interest in the management of musculoskeletal disorders of the spine and extremities. He has dedicated 20+ years of practice towards groups of people who have orthopedic injury or have been negatively impacted by dizziness, disequilibrium, unsteadiness and falls.

Since beginning his tenure at Jewish Senior Life, Chad has received certifications in LSVT Big for the treatment of clients with Parkinson’s disease, and in Vestibular Rehabilitation Therapy and the use of Videonystagmography (VNG) for evaluation of the dizzy patient.

Kathy Stoklosa, OTD, MPA, OT/L, Doctorate Occupational Therapist

Kathy brings more than 30 years experience to Outpatient Rehabilitation. She has worked in many areas including hospitals, outpatient clinics, and home care, and has worked closely with orthopedic and plastic surgeons in the area. Her clinical skills are highly developed for working with individuals with orthopedic, burn, trauma, post-surgical and neurological conditions including stroke, brain injury, Parkinson’s disease, multiple sclerosis, and ALS. Her most recent clinical work included working in home care for Lifetime Assistance and as Chief of Occupational Therapy for UR Medicine. 

Kathy is a clinical assistant professor at Nazareth College. She has a Bachelor of Science in Occupational Therapy and an M. P. A., Public Administration with Health emphasis from SUNY College at Brockport, and an Occupational Therapy Doctorate from Rocky Mountain University of Health Professions. 

Margaret Zlotnick, Speech Language Pathologist

Margaret is a graduate of Nazareth College and Boston University and has a Master of Science degree in Speech Language Pathology and Certificate of Clinical Competence in Speech Language Pathology. She has professional certification in LSVT Loud.

She brings 20+ years of clinical experience to our outpatient rehabilitation program. She specializes in the treatment of adults with neurological disorders who have difficulty in the areas of speech, language, and cognition. She has extensive training in the diagnosis and treatment of swallowing disorders. Margaret is a certified Endoscopist, providing Fiberoptic Endoscopic Examination of Swallowing (FEES), to aide in the diagnosis and treatment of these swallowing disorders. Margaret also utilizes biofeedback assisted swallowing treatments (sEMG) and is trained in Modified Barium Swallow studies (MBS). She is a member of the American Speech and Hearing Association (ASHA) and Greater Rochester Area Medical Speech Pathologists (GRAMS).

Jordan Nolan, DPT, Physical Therapist

Jordan Nolan is a staff physical therapist in the outpatient rehabilitation department. Jordan graduated from Daemen College in 2014 with a Doctorate in Physical Therapy Degree. After receiving his degree he worked for three years in the Rochester area in an outpatient orthopedic setting treating patients suffering from musculoskeletal and neuromuscular conditions. He has taken continuing education courses in the McKenzie Method for spine evaluation and treatment, and courses on treating vestibular/vertigo pathologies. He enjoys getting to know each patient individually and treating the entire person with the goal of getting the individual back to doing the things they personally enjoy.

**We have partnered with Rochester Rehabilitation’s DriveOn program to help seniors drive safely, comfortably and stress-free. Our occupational therapists work with DriveOn’s experienced driver specialists to evaluate driving ability and create a customized driver training program to help older drivers maintain their driving independence.
