Adult Day Health Care
The Adult Day Health Care center at the Jewish Home is a medical model program that serves adults with special physical and psychosocial needs. Encouraged by staff to increase their self-esteem, social interactions and independence, members who participate in Adult Day Health Care are better able to remain with their caregivers in the community.
What You Can Expect
We are one of the largest providers of adult day health care services in the greater Rochester area, assisting members on weekdays from 8 am – 4pm, with evening sessions coming soon. Our Adult Day members look out for one another—just like family. We also welcome those who seek activities and social interaction with peers. As part of the Jewish Home, Adult Day members have access to additional programs to stimulate mind and body.

Our staff has received certified dementia and memory care training through the Alzheimer’s Association. Services include the following:
- Assistance with daily medication
- Personal care and hygiene
- Nursing
- Physical, speech and occupational therapies
- A hot noon meal that can accommodate special diets
- Foot care
- Activity programs
- Transportation within Monroe County

Physician House Calls
We’re dedicated to caring for older adults throughout our community. Our unique Physician House Calls practice has a staff of physicians and nurse practitioners who provide a convenient, manageable way for older adults to get the high-quality medical care they deserve in the comfort of their own home. Our program includes the following:
- In-home primary care by physician and nurse practitioner
- In-home urgent visits for medical problems
- Coordination of care with specialist physicians and home health care services
- Team approach with social worker and psychiatric nurse practitioner available as needed
- Review of medications
- Assessment of abilities to perform daily activities
A safe and secure option for:
- Home bound, non-driving adults over age 65, residing in the zip codes we cover
- Our program Is not Medicaid-eligible, but we do accept most other insurances

Outpatient Rehabilitation
The Atkin Center for Outpatient Rehabilitation provides multi-disciplinary rehabilitation services for adults of all ages. With a full spectrum of therapy services, as well as specific treatments for a range of health issues, patients benefit from one-on-one personalized therapy plans that help them return to their highest level of independence. Our experienced therapists work as a team to provide comprehensive support. We offer speech and language, physical, and occupational therapies in one location to support recovery and build patient strength and confidence to get back to fuller functioning and living.
Regaining Independence
We see patients whose abilities have been affected by the following:
- Stroke
- Parkinson’s Disease
- Head injuries
- General neurological disorders
- Imbalance/gait disturbances
- Head and/or neck surgeries or injury
- Multiple Sclerosis
- Dizziness, disequilibrium & vertigo

- Lee Silverman Voice Treatment (LSVT) Big and Loud*

* LSVT Big and Loud is a treatment that helps those recovering from Parkinson’s and other neurological disorders regain strength and improve clarity in their speech. **We have partnered with Rochester Rehabilitation’s DriveOn program to help seniors drive safely, comfortably and stress-free. Our occupational therapists work with DriveOn’s experienced driver specialists to evaluate driving ability and create a customized driver training program to help older drivers maintain their driving independence.

Living Well Companion Care
Living Well Companion Care is the perfect solution for those who aren’t ready to leave their home but need support to stay at home. Our well-qualified, carefully screened team can make your daily life easier and safer by providing compassionate, non-medical assistance in the comfort of your own home. Our personalized and affordable services are available seven days a week, 24 hours a day, with a low minimum of two hours per shift.
Companion Care
Companions can help offer support and friendship that makes daily tasks easier. Having consistent and reliable support can improve your quality of life by reducing stress and worry. Living Well Companion Care offers non-medical, hands-off assistance when you need it. We can provide support to you in the comfort of your home, helping with:
- Running errands
- Light housekeeping
- Pet care
- Medication reminders
- Safety and supervision
- Light meal preparation
- Grocery shopping
- Laundry
- Hospital to Home - We can assist you in getting settled home after discharge from the hospital or rehab.
All our Companions undergo comprehensive background checks (criminal, DMV and drug screening) and provide support with a range of activities.
Transportation
Living Well Companion Care offers reliable local transportation to activities such as:
- Medical appointments
- Hairdresser
- Social activities
- Shopping
Our Transportation Services are intended for those that can get in and out of a car on their own, with or without the use of a walker or cane. Companions can use your car or theirs.
Housekeeping Services
Living Well Companion Care’s General Housekeeping services include sweeping, vacuuming or wiping down areas in: Kitchen, Bathroom, Bedroom, Living Room or General Areas.
Living Well Companion Care’s Heavy Housekeeping services take longer to complete and include a much more thorough cleaning, including:
- Removing scale and soap scum
- Cleaning behind appliances
- Deep dusting, including baseboards, doors, blinds and window coverings
We match our Companions to you based on a personalized service plan that considers your lifestyle, personality, medical conditions and types of assistance you need.

When appropriate, we will conduct a complementary in-home evaluation (approx. 45 minutes) where will learn about your lifestyle, medical conditions, preferences and more to evaluate the kind of assistance that would be must helpful.
Marian’s House
Marian’s House offers daytime respite for people with early to mid-stage Alzheimer’s and other related dementias, offering meals, activities, supervision and specialized programming in a warm, residential setting.

A Welcoming Memory Care Retreat
On behalf of The Farash Foundation, Marian’s House is named in honor and of blessed memory of Marian M. Farash. The house, located at 2980 S. Clinton Avenue in the town of Brighton, is specially designed for both one-on-one interaction and group activities and includes these amenities:
- A large open kitchen and eating area so guests can be a part of meal preparation
- A family room with ample space for cultural art activities, such as musical performances and painting
- Small quiet areas for peaceful relaxation
- Enclosed sunroom provides a bright, cheerful, plant-filled space
- A fenced-in yard and garden that provides a safe outdoor walking area, picnic space, and outdoor lawn games
- Full-day services include alight breakfast, full lunch and dessert.

Activities at Marian’s House center on everyday life, such as cooking, gardening, exercising, reading, and games. Guests are encouraged to participate as much as they are able and willing. A program manager with expertise in the field of memory care supervises all activities and services*.
Marian’s House guests can come for either all or part of the day. Hours of operation are Monday through Friday, 8:00 am – 4:30 pm, except holidays. Caregivers must reserve space for the day(s) they wish their loved one to attend.
* Marian’s House does not provide medical or health care services.
