The Jewish Home of Rochester offers skilled nursing care for those individuals that need full time care on a long term basis. Care can mean physical or medical, but there is also a need to provide emotional care and support. Whether it’s sharing stories about children and grandchildren, encouraging participation in cultural activities or simply saying hi with a smile, our caring culture is at the core of who we are.
Our highly trained, on-site medical staff members are employees—which enables them to provide consistent care and helps foster deeper bonds between our residents and staff. What’s more, our interdisciplinary team works collaboratively to provide a total-care approach, customized for each resident’s needs.
Our skilled nursing living options include nine “small homes” called the Green House Cottages and the newly renovated Jewish Home Farash Tower. All our long term care options have private bedroom suites with personal bathrooms and beautiful, naturally lit spaces for gathering, dining and socializing.
Our beautiful living spaces are only the beginning. Residents enjoy varied, nutritious dining options – with an emphasis on providing choices to meet personal preferences. Our talented recreational therapy teams provide a variety of programs such as music therapy, gardening, open creative arts, live performances, day trips and more to keep residents engaged and involved.

Many people require skilled nursing and rehabilitative services following hospitalization for surgery, infectious disease conditions or other complex health conditions. It is our goal to provide the best patient experience possible and to assist you in a speedy and effective transition back to your community. We offer a range of specialized care programs to patients of all faiths and ethnicities. Each program is supported by a focused, highly qualified on-site medical staff. And because our highly skilled, certified therapy staff are our employees—not temporary or contract staff—we’re able to provide a unique level of consistency, stability and quality for our clients.
If you’ve suffered a heart attack, had bypass surgery, valve replacement surgery, or struggle with endurance, you need specialized care to get you back to your best. The myHeart® Cardiopulmonary Rehabilitation Program provides short-term, sub-acute rehabilitation to rebuild the strength and confidence you need to bridge the gap between hospitalization and home.
Our interdisciplinary care team provide support to assist in recovery from stroke, hemorrhaging stroke and transient ischemic attacks (TIAs). Stroke recovery care focuses on maximizing the patient’s health and independence. Our physical, occupational and speech therapy is available seven days per week.
Specialized Short-Term Rehabilitation
Whether you’ve had elective joint replacement surgery or need to recover from a traumatic injury, our orthopedic rehabilitation program can help get you back on your feet.
Many people require skilled nursing and rehabilitative services following hospitalization for surgery, infectious disease conditions or other complex health conditions. Many of these people require IV medications, complex wound care, close nursing and medical monitoring, and patient and family training to learn how to manage new medical devices, medications and diseases.
Our Diabetes Center of Excellence is focused on improving overall care for our patients and getting you back to your best self. Since many short-term rehabilitation patients are currently living with diabetes, we have resources available to help manage this disease and prevent it from affecting other diagnoses.
We have a full medical team working to get you back to your daily routine and, most importantly, ensure you don’t return to the hospital. Onsite personnel include board-certified physicians in geriatrics, internal medicine and palliative care, geriatric nurse practitioners, and physician assistants. Along with a 24-hour nursing staff and certified physical/occupational therapists, our entire medical team collaborates to develop plans focused on your unique needs and goals. Located on a secured wing of the Jewish Home Farash Tower, the Jewish Home’s Neurobehavioral Rehabilitation Program is a specialty program designed for individuals who have traumatic brain injuries, neurologic impairments, or certain behavioral health disorders. This program is best suited for patients who demonstrate problem behaviors that are low in frequency and low to moderate intensity.
Treatment plans are individualized and focus on developing behavioral management strategies. We use these strategies, as well as a stable activity pattern, to promote a patient’s independence and personal awareness. We work closely with patients and their families to achieve maximum health and functional ability. Our treatment goal is to have patients be at a level where they successfully manage a daily routine within the community after discharge.
We provide a high staff to patient ratio so we can best support our patients. Our program staff, including nurses, therapists, behavioral specialists, and psychiatrist, are trained in behavior management techniques, neurobehavioral intervention, data collection, and advanced rehabilitation skills.
